Single Unified Safeguarding Review
The SUSR aims to create a single Review process which incorporates a multi-agency approach where the criteria for one or more of the following reviews is met:
- Adult Practice Review (whether concise or extended);
- Child Practice Review (whether concise or extended);
- Domestic Homicide Review;
- Mental Health Homicide Review; and
- Offensive Weapons Homicide Review;
The SUSR aims to:
- eliminate the need for families to take part in an onerous and traumatising cycle of information-giving and waiting for the conclusions of multiple reviews;
- ensure the subject and family are at the heart of the process;
- use the insight and learning gained from the Review process to deliver positive change in
practice to prevent future harm; - provide national support to the Safeguarding Boards which fulfils a co-ordination/operational role to oversee the end-to-end process via the SUSR Co-ordination Hub;

Child Practice Reviews
The criteria for child/adult practice reviews are laid down in the Safeguarding Boards (Functions and Procedures) (Wales) Regulations 2015. The arrangements came into force from 6 April 2016. The purpose of a review is to identify learning for future practice. It involves practitioners, managers and senior officers in exploring the detail and context of agencies’ work with a child and family.

Adult Practice Reviews
Safeguarding Adult Practice Reviews are carried out on behalf of the North Wales Safeguarding Adults Board. They are a way for all partner agencies to identify the lessons that can be learned from particularly complex or difficult Safeguarding Adults cases and to implement changes to improve services in the light of these lessons.
