Welsh Government Adult Safeguarding Data set Requirements April 2016

A new adult safeguarding standard data collection return based on the legal duties to report and enquire detailed in the statutory guidance in relation to Part 7 of the Social Services and Well-being (Wales) Act 2014 has been developed. The data items in this return are detailed below and must be collected on commencement of the Act in April 2016. Cyhoeddi AAYEP Sir Ddinbych 1

Dyma Adroddiad Adolygiad Estynedig Ymarfer Plentyn cyntaf i gael ei gyhoeddi gan Fwrdd Diogelu Gogledd Cymru o dan y fframwaith newydd. Mae’r adroddiad ar gael ar Dudalen Adolygiadau Ymarfer Plentyn.

 

Cafodd yr Adolygiad Estynedig hwn ei gomisiynu yn Rhagfyr 2014 gan Gadeirydd Bwrdd Diogelu Lleol Conwy & Sir Ddinbych, yn dilyn marwolaeth plentyn 5 wythnos oed oedd yn adnabyddus i Wasanaethau Cymdeithasol Sir Ddinbych ac ar y Gofrestr Amddiffyn Plant.

 

Mae Adroddiad i’r Adolygiad Estynedig wedi ei yrru i Lywodraeth Cymru yn Mis Medi 2015 ac o dan y rheoliadau, mae Tim Diogelu Llywodraeth Cymru ynghyd âg adrannau eraill o Lywodraeth Cymru a’r Grwp Arolygaeth yn cysidro os oes angen gweithredu pellach. Nid oedd angen gweithredu pellach.

 

Bu i’r adolygwyr annibynol adnabod sawl enghraifft o ymarfer da, fel perthynas weithio dda rhwng yr asiantaethau a’r teulu. Ond thema gyson o fewn yr adolygiad oedd prosesau trosglwyddo broblemus a newid personel oedd yn gweithio hefo’r teulu. Un prif neges i asiantaethau oedd pwysigrwydd bod gan staff fynedfa i oruchwyliaueth ffurfiol a safonnol.

 

Mae cynllun gweithredu wedi cael ei baratoi ac yn cael ei fonitro gan Fwrdd Diogelu Plant Gogledd Cymru er mwyn sicrhau bod dysgu ac adlewyrchiad wedi cymeryd lle gan yr asiantaethau oedd yn gweithio gyda’r teulu.
